Registered Nurse (RN) - Adult Day Healthcare Centers

Adult Day Health > Rhode Island > Clinical Operations > Registered Nurse

Elmwood Adult Day Health Care Center is seeking a compassionate and skilled Registered Nurse (RN) to provide direct nursing care to our participants, with a special focus on supporting individuals affected by cognitive impairments. The RN will play a vital role in promoting the health and well-being of participants by administering medications, conducting assessments, and supporting the development of personalized care plans.

Key Responsibilities:

  • Administer medications and treatments in accordance with individual care plans.
  • Conduct nursing assessments to monitor and evaluate participant health.
  • Ensure prompt and effective care delivery tailored to participant needs.
  • Maintain accurate and up-to-date medical records for all participants.

Assessment and Evaluation:

Conduct comprehensive assessments of patients' health status, including vital signs, medical history, and physical examinations.

Develop and implement individualized care plans based on assessment findings and patient goals.

Care Coordination and Planning:

Assist in developing and updating individualized care plans.

Conduct weekly care planning meetings in collaboration with the Nursing Director.

Work with interdisciplinary team members to address changes in participant conditions.

Qualifications

  • Education: Associate's, Bachelor's Masters or Doctoral degree in Nursing from an accredited program.
  • Licensure: Current, valid and unencumbered license to practice Registered Nursing (RN) in the State of Rhode Island and Providence Plantations.
  • Experience: Minimum of two (2) years of nursing experience in elder care, community health or long-term care settings.
  • Skills: Strong clinical assessment and therapeutic skills; Excellent communication and interpersonal skills.
